Not everyone is able to complete their Direct Debit Form online. You can download a copy of the Direct Debit Form here in order to print it.

Completing Your Premium Funding Documents

When setting up your funding agreement, you will need to review and complete two important documents:

♦  Your Premium Funding Agreement with attached Payment Schedule.

♦  Your Direct Debit Authority (or ‘DD’).

Both documents will be sent via email as a pdf and can be securely and digitally signed and completed through the browser on your web connected device. A copy of your signed documents will automatically be sent to you on completion.

We are also able to take your Direct Debit Authority instruction by phone, provided we have recorded the call and have obtained the correct information.

The setup of your Direct Debit requires us to provide you with multiple levels of notification as to the amounts and expected debit dates, for new authorities, and confirmation of any direct debit authority change instructions, to ensure there are no surprises.
 
Your rights and obligation under a Direct Debit Authority are important, and are set out on the ‘Terms and Conditions’ page.
 
Changing or Cancelling a Premium Funding Agreement. Please review the Terms of your Funding Agreement and contact our office by Telephone or Email to obtain your settlement amount. Your change or cancellation will become effective once we send you written acknowledgement, which is via the email address you have provided us.
 
Changing or Cancelling a Direct Debit Authority: Your Direct Debit Authority is separate to your Premium Funding Agreement. As per the Terms and Condition above, you may change or cancel your Direct Debit Authority with 7 days notice, by contacting our office by telephone or email. Your change or cancellation will become effective once we send you written acknowledgement, which is via the email address you have provided us.
